​ANIMAL SURRENDER/OWNERSHIP TRANSFER AGREEMENT

    I, the undersigned, as owner, do hereby transfer permanent title, ownership and possession, without reservation or right of rescission, of the following animals/property to the Minnesota Federated Humane Societies:
    ​I represent, in good faith, that I am the owner of the above-described animals/property and have legal authority to transfer ownership. I transfer ownership and possession of said animals/property by surrendering them to the Minnesota Federated Humane Societies. I fully release any claim to ownership or possession, and I hereby waive any right of rescission or avoidance of this agreement.
