    We, Minnesota Federated Humane Societies, the undersigned, as owner, do hereby transfer permanent title, ownership and possession, of the following animals/property to:

    I represent, in good faith, that I am the owner of the above-described animals/property and have legal authority to transfer ownership. I transfer ownership and possession of said animals/property.
