ANIMAL OWNERSHIP TRANSFER AND SURRENDER AGREEMENT TO MFHS

    ​I, the undersigned, as owner, do hereby transfer permanent title, ownership and possession, without reservation or right of rescission, of the following animal(s) to the Minnesota Federated Humane Societies: 
    ​I represent, in good faith, that I am the owner of the above-described animal(s) and have legal authority to transfer ownership. I transfer ownership and possession of said animal(s) by surrendering them to the Minnesota Federated Humane Societies. I fully release any claim to ownership or possession, and I hereby waive any right of rescission or avoidance of this agreement.
